# Add an ungraded survey

Ask for opinions or reflection without treating responses as correct or incorrect.

## Before you start

- Decide whether you need a choice question, a rating scale, or a written response.

- Do not assume a survey is a central reporting or data-collection service.

## Step by step

1. Choose Add block → Assessment → Survey.

2. Enter the survey title and use Add question for each item.

3. Choose a choice, Likert, or open-text question and write its prompt.

4. Edit choices or scale labels so learners understand what each response means.

5. Preview and complete the survey as a learner.

6. Check the exported course in its intended delivery environment. Use a separate survey service if you need saved responses or reporting.

## What to expect

The survey is ungraded and does not count toward the quiz score. Responses exist only in the current player session: they are not stored persistently or sent to the author or LMS.

## Tips & troubleshooting

- Use a graded Quiz when the question has a correct answer that should affect completion.

- Avoid asking for sensitive personal information unless you have established an appropriate collection and reporting workflow.

- If you need aggregated feedback, use a suitable external survey workflow and explain how learners should access it.
